CJSA REQUIREMENTS FOR 2009

1.  You MUST submit a copy of your child’s birth certificate with their registration, even if they have played in previous years.

2.  Anyone who wishes to be a head coach, an assistant coach, a scorekeeper, etc., MUST mail a Completed Volunteer Form; a Copy of your Drivers License; and a One Inch by One Inch color photograph of yourself to 47 Arbutus Street, Middletown, CT  06457.  You will not be allowed to assist the players until you receive a pass from the league.  Click HERE for the 2009 Volunteer Form.
